HPH CONNECT supports efficient communication with the ease of sending HIPPA-protected referrals to HPH Specialists through the mobile app.

HPH CONNECT allows for the personalization of your directory and effortless browsing of all Humboldt Park Health Specialists.

With HPH CONNECT, sending a referral to an HPH Specialist and a text message to your patient will only take a few minutes saving you valuable time. The selected HPH Specialist will be alerted and the patient contacted.

The HPH CONNECT referral app was designed to best support communication between Community Physicians and Humboldt Park Health Specialists for a safe and efficient way to provide care throughout the continuum.

*All referrals are of course still subject to the Compliance and Privacy regulations referenced in the consent approvals which will appear in the sign-in to the App. Improper practices such as kickbacks for referrals, referrals to relatives, and other similar practices, continue to be illegal under federal Fraud and Abuse Laws whether through HPS Connect or any other referral mechanisms.
