Partners Connected Health PGHD Connect is a cloud-based digital health platform that provides a link between patients and providers. Use the PGHD Connect app to share personal health data with your care team/provider seamlessly and securely using your own consumer or medical grade device. The PGHD Connect app supports a variety of devices across four data types including Blood Pressure, Blood Glucose, Weight and Activity. Also use the PGHD Connect app to view your personal health results. Users of the PGHD Connect app must be invited by their provider to share their data. This app integrates with the Health app
