ModMed has been an enormous disappointment as an electronic medical record, and switching from NextTech to ModMed was honestly one of the worst decisions I’ve made for my practice. I am counting down the time until my contract ends so I can switch back.
The system is frustratingly inefficient in ways that should not exist in a modern EMR. For example, every single time I see a patient who has had a cholecystectomy, I have to manually type in “cholecystectomy” and then sort through what feels like 30 different diagnosis options. If a patient tells me they had back surgery, it is the same story. I have to freehand it into “other” rather than simply selecting from a customizable list of commonly used diagnoses. That is absurd. This is not how an EMR should function in this day and age.
An EMR should improve workflow, not slow it down. It should be intuitive and customizable. I should be able to create and save a list of my most common diagnoses and past surgical history items instead of wasting time hunting and pecking through endless irrelevant choices like I am on some kind of scavenger hunt every time I see a patient.
The reporting is also terrible. Pulling useful data is far more difficult than it should be, and overall the platform feels clunky, rigid, and far less capable than NextTech. Nearly every part of the experience feels like a downgrade.
The photo management is especially pathetic, which is almost unbelievable for an EMR that markets itself specifically to plastic surgery practices. It is beyond lacking. Even the image quality is a problem. If you use the EMR itself to take patient photos, the images may be acceptable for basic chart documentation, but when you try to use them for your website, marketing, or any other professional purpose, the quality is grainy and unusable. That alone is a major failure for a plastic surgery-focused system.
On top of that, there are not even thumbnails in the mobile app. So if you are trying to find specific patient photos from your phone, you are left guessing which images you need to open because there is no visual preview. In a specialty where photography is central to patient care, outcomes tracking, and marketing, that is not a minor inconvenience. It is a glaring flaw.
In short, ModMed has been vastly inferior to NextTech in usability, efficiency, reporting, and photo management. I would not recommend it to any plastic surgery practice that values speed, customization, functionality, or high-quality image organization.

The EMR is so segmented and awkward that a single note can require tens of clicks to complete it.
