One shared plan for family life: health records, medications, tasks and AI.
Life and healthcare, finally on the same page.
HomeTeams brings family, friends, neighbors and care professionals into one shared space, so everyone knows what's happening and how to help.
You've been carrying a lot, and you don't have to carry it alone. Caring for a parent, supporting a child, managing a health condition — wanting help isn't weakness. It's human. Even the best teams need a game plan and people who show up.
Caregivers who go it alone average 22.8 hours of care work a week (AARP & NAC, 2025). With a HomeTeam, that load is spread across many hands.
BUILT FOR REAL LIFE
Caring for aging parents. Managing a chronic condition. Recovery and post-hospital support. Family scheduling and everyday household coordination.
YOUR HEALTH RECORDS, PULLED IN FOR YOU
Verify your identity once and HomeTeams gathers your medical records for you — hospital stays, the urgent care visit you forgot about, labs, clinical notes and medications, from the places you've been treated. New records keep arriving as care happens, so the picture stays current. Ask Coach to explain any of it in plain English.
Add your own notes and documents, and choose which teammates see what. Connect Apple Health for steps, sleep and weekly activity trends. Keep insurance cards and forms in the Resource library. When you need a real person, reach a nurse from the Health tab, day or night.
SET UP IN MINUTES
Answer four questions out loud and Coach builds your team, or type them if you'd rather. Name the team, add the first appointment, send invites.
ONE SHARED PLAN
Team Home is the feed everyone sees: what happened today, what's coming up, who has it covered. As Team Captain you decide what each teammate can see, and can switch teams if you support more than one household.
TASKS THAT ACTUALLY GET DONE
Create a task — we call them actions — for errands, appointments, rides, meals, medications or quality time. Choose who it's for and who does it, set a date or leave it open, and sort with color-coded categories. Filter by category, status or day, or see the week at a glance. When a task looks like a medication, HomeTeams sets it up with dose, strength and reminder times.
COACH, YOUR MENTOR AND GUIDE
Coach is the AI at the heart of HomeTeams. It answers the question in front of you at 11pm, in plain words — Coach knows the plan, the medications, and who to pull in. Ask by voice or text, open the daily digest to see what changed, and let Coach turn guidance into steps your team can pick up.
HOMETEAMS WALLET
Care costs money and it rarely falls evenly. Link a card you already use and cash back accumulates passively at participating merchants, tagged to your team's goal. Relatives can contribute directly to shared Team Funds. Real cash, not points, not coupons — with a clear view of who has given what.
BRAE PROGRAMS
Structured mind, body and soul health plans: living with a chronic condition, building the mental toughness caregiving demands, or working through forgiveness. Short daily practices, gentle accountability, and a team that shows up with you.
LIVING LEGACY
Some things should outlast the calendar. Living Legacy captures a life story in the person's own voice, one gentle question at a time, with Coach guiding. Old photos are restored and kept in a private family vault.
STAY CONNECTED
Message the whole team, a small group, or one person, without endless group texts. Notifications separate what needs attention from what's handled.
Most apps help one person stay organized. HomeTeams helps people show up for each other, and turns caring into coordinated action.
Build your team. Care together.
HomeTeams is not a healthcare provider or a medical device, and does not diagnose, treat or prevent any condition. Coach and the nurse line provide general information only and are not a substitute for care from your own clinician. HomeTeams does not provide emergency services. In an emergency, call your local emergency number.
