To support your recovery, your personalized home exercise plan is available on our easy-to-use app, featuring:

• At-home exercise programs designed by your Hartford HealthCare Rehabilitation Network provider • Easy-to-follow exercise videos with clinician-led voice instruction • Daily progress tracking to give your care team insight into your recovery • Access in-app chat support from a dedicated remote Care Navigator to assist with your exercises and answer your questions (*available only to eligible users; ask your provider if you qualify for this at-home support service) • Apple HealthKit integration to sync your health data to the HHC Rehab Connect app
