The MND PRISM app assists you in monitoring your progression and coordinating the many appointments, services and medications a person living with MND has to juggle. It is full of practical tools to stay organised, assist carers and family members with logistics and track your symptoms. With the MND PRISM app you have the option of self-monitoring your disease progression and assessing your overall health and wellbeing using the SHOWMe Diary. Additional capability in the app assists in collating all contact information for your clinical and care teams, tracking appointments, medications and changes in prescriptions and your equipment.

In order to use the app, you must register with the MND and ME Foundation and obtain a user account for using the app. Register here: https://www.mndandme.com.au/mnd-me-app

After registration you will receive an email with the login details to login to the app. After logging into the app, you can set up your profile and begin to enter data into the different modules, view your data and generate reports.

Registration directly via the MND PRISM app is deliberately not intended. It takes place via registration on the MND & Me foundation website.
