KP Health Ally app from Kaiser Permanente helps patients enrolled in diabetes or hypertension Remote Monitoring programs share device readings with their care team. After enrollment by a KP clinician, you can sync and manage readings using your KP.org login and participate in multiple remote monitoring programs from a single app.
