Form Health is great. So thankful I was referred to them. I am so thankful for my Medical team.. Kaci, Chivas and Lara. They are so caring, understanding, supportive, and always there for me. I mess up regularly, but they are right there to get me back on track.. again and again. If you haven’t tried Form Health, you’re missing out.

What ever they did on the most recent update the rescheduling is impossible to do. The screen won’t pop up and stays stuck at the bottom of the screen with no way to bring it up. Makes me have to make several attempts to reschedule because of how long support services takes to respond to reschedule requests.
