Log my Care

Log my Care is a person-centered care management app for care teams and providers. Carers can record delivered care, including meals, clinical support, activities, incidents, and handovers, with options to upload photos and short videos.

The app provides secure access to care documents, support plans, daily tasks, goal and progress tracking, and specialist features such as Positive Behaviour Support (PBS). With the Rostering add-on, carers can view schedules, planned absences, and clock in or out with a single tap. Log my Care works with the Care Office platform and requires provider sign-up.
