There's an optional "Snap to Element" feature to help align guides to your elements while dragging. There's also a "Snap To Grid" feature that helps create or align guides to a grid of your choosing.

Like others, at first I thought it wasn't working (probably due to its age), but as the reviews were informative: to make it work with a web page you had open before installing the extension, you just have to refresh the page. Works like a champ and very useful!
