Ordinary pill apps send notifications, and notifications are silenced by Silent mode and by Focus. Dosely schedules a real system alarm, the same kind your Clock app uses, so it rings when it matters.

If you would rather have something quieter for a dose that lands in a meeting, you can set that medication to notify instead. Both options are there, and the app never pretends one is the other.
