HOW IT WORKS

2. Speak naturally. Chartflow transcribes 18 languages and handles mid-sentence code-switching.
3. Review the draft. Every section is editable, expandable, and traceable back to the transcript.
4. Sign when it's right. Nothing is finalized until a licensed clinician signs it.

BUILT FOR REVIEW, NOT AUTOPILOT

Chartflow drafts; clinicians decide. Notes are marked as AI-generated drafts and require your clinical review before signature. A pre-signature chart review surfaces missing items, inconsistencies, and gaps while you are still in the home — but it never signs, and it never files anything on your behalf.

BEFORE YOU RECORD

Chartflow shows you exactly how visit audio is handled — including that it is processed by third-party AI services under HIPAA Business Associate Agreements — and asks you to acknowledge it before recording begins. Obtaining patient consent to record, as required by your state's law, remains your responsibility as the clinician.

REQUIREMENTS

Chartflow requires a Chartflow account with an active subscription, and is intended for use by licensed clinicians and the home-health agencies that employ them. It is not a consumer health or wellness app.
