Daily Survey - You configure the daily survey to track exactly what you want. When something new comes up, easily add it to your survey and Chronic Health will ask about it automatically until it resolves.

Custom Configuration - You configure exactly the symptoms, treatments (with side effects), and illnesses (with symptoms) you want, and measure them on one of four scales: (1) severity, (2) count, (3) yes/no, and (4) 0-10 pain.

Photos and notes - Add context to your daily journal by adding photos and notes. Share the relevant photos with your doctor at your next visit.

Visual Calendar - The visual calendar lets you see your health at a glance, with quick filters to easily dive into the conditions on your mind.

Appointments - Keep track of all your medical appointments, write down topics you want to cover, take post-appointment notes, and see your whole appointment history.

Reports - Quickly generate a report covering the health items and date range you care about. Dive into graphs over time, export a PDFs to share with providers, and export spreadsheets for further data analysis.

Tags - Each item can be tagged in a group for quick filtering when generating reports or viewing the calendar.

Privacy - This app requires no login and your data stays on your device.
