Capture everything that matters: Document title, date and time, GPS or manual location, severity, incident type, injury details, immediate actions taken, photos with annotation, audio memos, structured witness statements with speech-to-text transcripts, chemical hazards, and weather — automatically pulled from Apple Weather when available, with National Weather Service as a fallback for US locations.

Investigate with the right tools: The RCA Hub puts your root cause methods in one place. Use 5-Why, Fishbone, Fault Tree, Timeline, or HFACS to understand what happened and why. HFACS surfaces automatically for military contexts and can be enabled for any investigation. If your organization uses something else, build a custom framework that fits your process.

Manage CAPA from start to finish: Create corrective and preventive actions, assign owners, track due dates, flag overdue items, measure effectiveness, and link new incidents back to prior CAPAs to detect recurrence and prove whether your changes are actually working.

Finalize investigations with integrity protection: When an investigation is complete, seal it with a SHA-256 cryptographic fingerprint. Finalized records are locked against silent edits — follow-on information is captured as amendments instead of overwriting the original.
