Care needs do not always happen according to plan. A person may be ready to leave hospital before ongoing home support is in place. A caregiver may suddenly become unavailable. A scheduled visit may be cancelled, or a family may need short-term help while a longer-term care arrangement is being organized.

Caredara is designed for these moments.

Through Caredara, clients and families can access qualified Personal Support Workers (PSWs), Registered Practical Nurses (RPNs), and Registered Nurses (RNs) who are part of the Caredara Care-Ready Network™.

Caredara focuses on rapid in-home stabilization rather than replacing ongoing home-care providers.

The goal is to help address the immediate care gap, stabilize the situation, and support a transition to an appropriate continuity-of-care arrangement.

Caredara may support situations including: • Hospital-to-home transitions • Waiting for ongoing home care to begin • Unexpected caregiver cancellations • Temporary caregiver gaps • Short-term changes in care needs • Family caregiver disruption • Other non-emergency situations where qualified in-home support is needed

Caredara provides care activation infrastructure designed to complement existing health-system, community, home-care, and family supports.
