Registering for the My Circle Health App:

• To use the My Circle Health App, you must already have an appointment scheduled at a Circle Health Group hospital.
• Registration also requires your 8-digit EMPI (patient number), which can be found on your My Circle Health email or SMS invitation or appointment letters.
• You’ll also need to enter the mobile number, email address, date of birth, and surname exactly as recorded in your patient record.
