CareConnectors Executive is a secure, HIPAA-compliant companion app that gives authorized health-plans, medical-group leaders, primary care physicians and health care providers real-time insight into the quality programs they manage. It also provides their members the ability to see their health visit information including but not limited to their recent assessments, lab results and visit history.

Who Can Use This App

• Members who have had an in-home or clinic health visit, or when they schedule a visit through https://myccmg.com website.
• Primary care physicians and nursing practitioners who manage services for their members or patients.
• Contracted health plans, medical groups that have a CareConnectors agreement to manage quality programs for their members.

Key Features

• Population Health Dashboard – Monitor compliance rates, gap closure progress, and visit completion in near real time.
• Member 360° View – Secure clinical snapshot for each member, including recent assessments, lab results, and visit history.
• Provider Portal – Primary-care physicians can review assigned members, document encounters, and close quality gaps on the go.
• Visit Scheduling (Coming Soon) – Members will be able to request in-home or clinic visits; providers can accept and manage appointments directly from the app.

End-to-End HIPAA Security – All data are encrypted in transit and at rest; no data are used for advertising or shared with third parties.

Privacy & Data Use

• Only the user’s name and organization-issued email are collected for authentication.
• We do not collect data for advertising or tracking, and we never sell or trade user information.
• The app contains no in-app purchases or paid content.
