navify My Care lets you securely access your health information from the care program that you were enrolled in by your healthcare organization.

You can: ● View data from your devices ● View recommended actions to take ● Get notifications for information from your healthcare provider ● Respond to surveys from your healthcare provider

To use navify My Care, first follow the link in the account activation email to set your password. Then log in with your enrolled email and this password.
