Need help to plan for later life? Life Transitions will help you to plan for significant events in your future such as retiring, moving home, changes in health, becoming a carer, making new friends, and many more.

This new app will help you think about what’s important to you, what resources you already have, and what you need to feel prepared for, and in control of your later life.

The app will guide you through a thought-provoking process, with lots of helpful prompts and resources, and help you to create an action plan for your future.

This app and its content has been designed and created by East Sussex County Council for the residents of East Sussex and West Sussex.
