SCH - Symptom Care at Home

The Symptom Care at Home (SCH) Mobile Application enables patients and their caregivers to interact and communicate with healthcare providers to enhance management of their symptom care. Patients can complete smart interactions, log symptoms, communicate with providers, and access a self-care library from anywhere.

Patients must be enrolled with a participating provider in order to use the SCH Mobile application
