The mobile Connecting Care app includes 2-3 minute automated/scheduled queries and motivational reminders to patients/family members via mobile app to:

• let the health care team know real-time/in the moment symptom reporting
• know what actions they have taken
• motivate them prompting to specific adaptive behaviors and self-correcting
• track and report their progress in real-time to their health providers
• recommend therapeutic options
• provide an accurate understanding of the patient’s situation, over days or weeks, resulting in tailored treatment plans and better outcomes
• gain more accurate data-based understanding of the patient’s situation and make adjustments as needed.
