Digital Tattoo Forms is a tattoo and piercing release form App that Artists and clients fill out before the procedure starts..

You can access clients and artists in real time. If you have multiple studios, on vacation or maybe live in different area you can log on and instantly see what your artist is tattooing, the price, description, location, how long they tattooed for and the customers details.

Maybe you think an artist isn’t on the up and up, you can call that client as soon as they walk out the door and do a personal survey with them such as how was your experience? How much were you charged? Do you feel you paid a fair price? Would you mind leaving us a Google or yelp review? All from the comfort of your office.

This app takes the guess work out and simplifies this step and the best part is it removes the stacks of paper forms piling up.

We’ve also just released a waiting list digital form for busy walk-in shops.

• For the unlimited forms experience, you can opt to choose the subscription plan: $19.99/ Month

• The payment will be charged to your iTunes account at confirmation of purchase.

• The subscription automatically renews unless auto-renew is turned off at least 24 hours before the end of the current period.

• Subscription may be managed and auto-renewal may be turned off by going to Account Settings after purchase.

• The account will be charged for renewal within 24 hours prior to the end of the current period.

• Any unused portion of a free trial period will be forfeited when the user purchases a subscription to that publication.
