Ascelus

Ascelus allows remote monitoring and management of your long term condition in partnership with your existing clinical team - from the comfort of your home. The Ascelus app is only available if the clinical team treating the patient recommends it as part of the care pathway.

The patient can do the following from the comfort of their home

1. Enter symptoms before the review with clinician. The symptoms can be entered on the application and we guide you fully on how to do it.
2. The blood test and other test result data from hospital is sent to Ascelus for you to view in application.
3. Ascelus sends you notification following review by your doctors and nurses. It also send reminders for appointments and tests.
4. You can see details of your future appointments.
5. You can view information posted by your clinical team on the application.
