Medibank Heart Health at Home app

Medibank Heart Health at Home is a phone-based cardiac rehabilitation program that runs over 6 to 8 weeks for eligible Medibank members anywhere in Australia.

During the program, more support is only a few taps away with the Heart Health at Home app, powered by Cardihab. Developed in partnership with the CSIRO, this evidence-based app provides advice and guidance to help you manage and keep track of your blood pressure, stress, medications, weight and other risk factors. Cardiac rehabilitation programs have been shown to reduce the risks of a heart attack and may prevent the need for more cardiac surgery.

You’ll receive: • Personalised one-on-one weekly coaching sessions with an experienced health professional • Support and counselling to make lifestyle changes and reduce stress • A program tailored to increase your physical activity

This app integrates with HealthKit/Health app (with your permission) to help you automatically keep track of your daily step count, so you don’t have to enter this in manually. As a user you can at any stage turn this feature on or off in 'Settings'.
