Docket® - Immunization Records

Docket makes it easier for residents in AK, DC, ID, ME, MN, MO, NJ, ND, UT, and WY to access personal and family immunization records. It partners with state health departments to let you view, track upcoming shots, review past immunizations, and share official immunization reports directly from official registries.

The app also supports SMART Health Card vaccine credential QR codes to facilitate travel to places with COVID-19 restrictions, and provides information about the Vaccination Credential Initiative.
