The official field application of the Ghana Health Service for community health
officers, nurses, midwives and patient navigators. PNCCS carries the household register, the home-visit record and the referral chain into the community, and keeps working where the network does not.

REGISTER HOUSEHOLDS AND CLIENTS
Enrol households and their members in the field, capture demographics and
contact details, and keep the register organised by community, sub-district,
district and region.

CONDUCT HOME VISITS OFFLINE
Complete visit checklists and clinical forms with no connection at all. Every
answer is saved on the device the moment it is entered and synced to the
national system automatically once the handset is back online.

CLINICAL DECISION SUPPORT AT THE POINT OF CARE
Guidance and alerts surface as a visit is recorded, so danger signs and
follow-up actions are flagged while the health worker is still with the client.
