ConsentForm

No more binders and crumpled paper. ConsentForm puts your clients' consent forms on your tablet or phone: health questionnaire, on-screen signature, traceability, then a generated PDF emailed to your studio. Designed with the practitioner's hand in mind.

WHO IS IT FOR? Tattoo artists, piercers, and permanent makeup artists. You select your services in your profile, and the form adapts: tattooing, piercing, or permanent makeup, each with its own specific information and traceability.

WHAT THE APP DOES
• A complete form: identity, contact information, date of birth, consents, image rights.

• A minor consent section automatically activated based on the date of birth, with a photo of parental authorization and ID.

• The client signs on screen; the practitioner's signature, entered once in their profile, is added to each form.

• The client also fills out a QR code on their own phone: a single-use QR code, or a permanent QR code to display at your workstation.

• Traceability in the client's file: inks or pigments (name, reference, batch number) via your digital cabinet; jewelry batch number or sterilizer code for piercings.

• A dashboard with your current files, badges that indicate a minor client or a health concern, and your activity statistics.

THE HEALTH QUESTIONNAIRE
Allergies, conditions or illnesses, medication, skin conditions, susceptibility to fainting: the client answers themselves, at their own pace. Each question leaves a record on the form, even if unanswered. From your profile, you choose which questions to ask.

YOUR DOCUMENTS
Each completed form becomes a time-stamped PDF, with your logo and both signatures, sent by email to your studio. With the premium version, your records can also be viewed in the app: search by client, filter by year, downloadable PDFs, and CSV export.

PREMIUM, OPTIONAL
Purchasing the app is all you need to work. A subscription, available for purchase, adds: access to your archives within the app, PDFs sent to clients upon request, scheduled appointments with reminders, your own post-session instructions sent to clients upon request, emails rewritten in your voice, feedback requests, alerts before your hygiene certificate expires, and a public mini-site with your name, including a gallery, online booking, and visitor statistics.

YOUR DATA
The website, API, and database are hosted by OVH. Photos taken during the final stages (inks, needles, jewelry) are not stored on our servers; they are only included in the PDF sent by email upon completion. No ads.
