Moje léčba od EUC

The application is intended for chronically ill patients who are in the Disease management program (DMP) of the EUC group.

The disease management program is intended for chronically ill patients who are being treated with one or more diagnoses from the group of cardiometabolic diseases: Type 2 diabetes mellitus, arterial hypertension, dyslipidemia, prediabetes. These patients are under the long-term care of a general practitioner or ambulatory specialist of the EUC group, who have drawn up a personal treatment plan for them.

In the application, you have available a digital version of your treatment plan, which serves as your personal "timetable".

The application will provide you with:
- control over compliance with the treatment plan,
- list of your recommended examinations,
- dates of ordered and performed examinations,
- target values ​​of your key health parameters (laboratory and measured values),
- overview of current results in the context of set target values,
- the possibility to record and monitor results from home measurements such as weight or blood pressure in the context of set target values,
- possibility to set notifications for home measurements or medication use,
- list of medicines from the treatment plan,
- automatic sending of measured values ​​from connected devices,
- control of daily activity for better motivation and treatment support.

In short, in the application you can see a comprehensive view of your treatment, your so-called timetable, thanks to which you will know when and where you are headed and what the goals of your treatment are. Adhering to and monitoring your treatment plan significantly contributes to the prevention of serious health complications and gives you and your doctor confidence that the treatment is taking place according to the most modern professional recommendations.
