Life Notes - Symptom Tracking is a free, volunteer-built Chrome extension for tracking chronic symptoms and daily habits entirely offline on your device. It supports rating symptoms on a 0-5 scale, logging meals, medications, notes, and sleep, and viewing daily trends to spot patterns.

None of your data is sent online. The app is ad-free and stores data only on-device. You can create custom symptom lists, back up data via Import/Export in Settings, and use it in English or French.

Very good app, but it needs improvements to be a great or awesome app. I'm trying to track serious medical events for a family member, as well as doctor appts, and medications. If we could add mini icons or symbols on the month view for each calendar day for 🩺 Doctor appointments or 💊 meds or 🏥🏨 for hospital visits or 😨🥵🤢 for minor to major medical events or sickness levels. It is a really good app so far, but it could be better with some tweaking. I would write more, but i'm out of room

Given what this is (a volunteer, ad-free project), it's great. The fact that it doesn't collect data and is relatively free form (ie let's you add your own titles and times) is great. As other users have noted, it can be clunky, but I'll take it any day over more sophisticated apps that charge me, collect my data, or push out ads. I do have a question for you, Seb - how do you transfer life notes data if you get a new device?? I need my historical log!

Quick and easy to enter data. Quick and easy to review data. No overwhelming descriptors for entries. You could track triggers, but I dont need to, and too much data gets in the way at dr appointments. I only need to show how I am doing. I can show a picture of the calendar with the days I added migraine highlighted. I can show my dr a count of how many times I took extra medication for anxiety per month, so I can demonstrate I don't abuse it, but still have sufficient need for it. Love it.
