Health Tracker is a symptom diary for anyone living with a long-term condition — and for the family members who help them keep track.

During the day, at times you choose, it asks a short list of yes/no questions about the symptoms that matter to you. Answering takes seconds. Weeks later, it turns all of it into a clear PDF you can hand to your doctor.

WHY IT EXISTS

Trying to remember six weeks of symptoms in a ten-minute appointment loses almost everything that matters. Notes on paper are easy to forget and hard to summarise. The pattern over time is what your doctor needs to see, and that pattern is exactly what memory is worst at.

CHOOSE WHAT YOU TRACK

Pick your symptoms from a list of common ones — fever, pain, headache, stomach ache, nausea, vomiting, diarrhoea, rash, itching, fatigue, cough, sore throat, breathlessness, joint swelling, trouble sleeping and more. Only what you pick gets asked.

Each answer takes the detail that suits it: a temperature for fever, body areas and a 1–10 score for pain, how many times for vomiting, a simple yes or no for the rest.

Change your list whenever you like. Nothing you have already recorded is ever deleted — it stays in your history and in your reports.

• Leads with how complete the data is, so nobody draws conclusions from half a picture
• A chart for each symptom, plus a full day-by-day log
• Says which symptoms were being tracked and when they were added, so a short history is never mistaken for a quiet one
• Share by WhatsApp, email, or print it

PHOTOS

Add a photo to any check-in — a rash, swelling, anything worth showing.
Photos are automatically deleted after 60 days.

SHARE WITH FAMILY

Invite someone by their Google address. Once they accept, they can see your check-ins and trends and send the report to your doctor on your behalf. They can never change anything you recorded. You can remove their access at any time, and it stops immediately.

Health Tracker is not a medical device. It does not diagnose anything, does not score or grade your symptoms, and does not give medical advice. It measures nothing on its own — it records what you tell it and presents it back plainly, leaving every clinical judgement to your doctor.

It is not a substitute for medical care but a helpful record for your doctor.
