URPS VDSI repérage

IDP SANTÉ develops digital solutions for the assessment, prevention, intervention and improvement of patient care. Among the tools developed by IDP SANTÉ, the tracking application for the URPS VDSI tracking, allows the identification of (pre)frailty in seniors aged 50 and over in the experiment organized as part of the Pr' program Agi'Lab implemented by Carsat Hauts-de-France. This identification is done using the Toulouse Gerontopole reference grid. Depending on the identification score, the beneficiary is considered robust, (pre)fragile - presenting risk factors for frailty - or fragile.
