In the realm of diabetes management for older patients, the challenge lies in ensuring they receive optimal pharmacological treatment. Despite the prevalence of diabetic kidney disease among older individuals, the existing clinical practice guidelines often lack a crucial element – personalized care.

Recognizing this gap, we present NephRASI Prescribe, an innovative Clinical Decision Support (CDS) tool designed to transform the landscape of prescribing renin-angiotensin-aldosterone system inhibitors (RAASi) for older patients with diabetes.

Older patients with diabetic kidney disease frequently fall short of receiving the most effective pharmacological interventions. Current clinical practice guidelines tend to overlook the importance of personalized care, which can significantly impact patient outcomes. Addressing this need, our research focuses on developing a CDS algorithm that not only considers existing evidence but also tailors recommendations to individual patient profiles.
