FormKiosk turns a tablet or a spare phone into a feedback terminal for your counter.

Someone taps a face, the screen thanks them, counts down and resets for the next customer. About four seconds, start to finish. No app for them to install, no account for them to make, nothing for your staff to reset between customers.

Built for a counter, not for a desk

The form fills the screen and the controls are sized for someone standing up in a hurry. Lock the device to the app so a customer cannot wander off into the browser. The screen stays awake all day.

It keeps working when the wifi does not

Answers are written to the device the moment someone taps Send, and they send themselves when the connection comes back. A dropout costs you nothing. The time recorded is when the customer actually tapped, not when the wifi returned, so your charts stay honest.

No password on the counter

The device shows a six-character code and you claim it from your phone at formkiosk.com/pair. What the device holds is access to one form and nothing else: it cannot read your responses back, see your other forms, or reach your billing. Unpair it from Settings whenever you like, without changing anyone's password.

It looks like your business

Your colours, your logo, your wording. Smiley or star ratings, multiple choice, free text, NPS. Build the form on the web, and the device picks up the changes.
