E-HealthCard HP(Mukhya Mantri

E-HealthCard HP (Mukhya Mantri) is a digital application developed by the Government of Himachal Pradesh to facilitate mass screening of Non-Communicable Disease (NCD) risk factors among individuals aged 30 and above. The app equips health workers (ANMs) with Android tablets to efficiently collect and manage health data at sub-center levels across the state.

Key features include supportive supervision and reporting through administrative hierarchies, disease prevalence estimation by geography, public access to personal health status via OTP for increased awareness, and automated SMS alerts for follow-ups of suspected or confirmed NCD cases. Available as both an Android mobile app (offline and online modes) and a web portal, E-HealthCard HP provides flexible data collection frameworks covering multiple health indicators, empowering evidence-based health program decisions and improving public health outcomes.
