teleHeart is a recovery companion app that aims to make your cardiac rehabilitation enjoyable, easy, & safe.

We work with your clinic to give you a connection to your nurses and care providers
no matter where you are. Working with your clinic, we bring you an individualized treatment plan that you can complete in your own time and at your own pace.

This app is designed to be adaptable to you. If some part of your recovery plan is troublesome, using our chat feature you can reach out to your nurses and they will work with you to change your plan at anytime!

Education is one of the three core components of heart recovery. teleHeart helps you by providing short, easy to understand lessons paired with simple quizzes that allow you to understand and manage your condition.

Track your exercise progress using a heart rate monitor and share your exercises with your clinicians so that they can view your achievements and track your improvement!

Keep a journal of your health records such as your weight, blood pressure, and blood sugar. These are some of the core pieces of information your health providers look at when making decisions about how best to help your recovery.
