Where is your source code? It is not in this description and it is not in the extension options page either. To be compliant with the GNU GPL v3 license (of the project you derived this extension from), source code must be provided along with your binary distribution.

1) No attribution / this is clearly a fork. 2) Doesn't work / hasn't fixed whatever API update has broken proxy invocation in the base project (simplest possible scenario of unauth'd local proxy).
