Core by integratedliving App is a personal health companion providing an integrated, online health service for individuals and their families. The app allows you to share health information with your personal care team and family and monitor, measure and promote health and wellbeing. It is

It is only available to integratedliving clients and their family members who have consented to engage the Core by integratedliving health service. To get started, clients or family members should contact their Core by integratedliving Personal Health Partner on 1300 782 896.

Core by integratedliving App provides you with easy access to your personalised health service wherever you are, helping to keep you living, feeling and being well.

Speak with your personal health care team and family via phone, video, chat or email. Keep track of your health using interactive assessments, surveys and remote monitoring sensors. Join one of integratedliving’s Wellness for Independence® programs, access your own personalised health library or make friends through a virtual ‘Let’s Have a Conversation’ morning tea.     

From the comfort of your home or when you’re on the go, Core by integratedliving App allows you to:   

* Interact with your personal health care team to help you achieve your health goals 
* Participate in virtual health and wellbeing services  
* Access your personal health library  
* Access your personal health management plan and financial statements  
* View your calendar of integratedliving services and reminders, reschedule or
request a new service 
* Measure and monitor your health status with integrated and secure remote
monitoring technology

* Provide feedback on integratedliving services  
* Stay up to date with integratedliving news. 

Please note, Core by integratedliving App may use background location services and continued use of this app may decrease battery life. Users of Core by integratedliving App are responsible for managing data usage within the limits of their mobile phone plan.
