You will need the email and password your clinic gave you to sign in.

WHAT YOU CAN DO

• Home dashboard See your day at a glance — greetings, quick actions, recent vitals, and alerts from your care team.

• My Health Log and review vitals such as blood pressure, temperature, and weight. Track activity and keep your medication list current.

• Messages Send secure messages and attachments to your doctors and nurses. Get notified when your care team replies.

• Appointments View upcoming and past visits, including date, time, provider, and visit type.

• Profile & documents Update demographics, insurance, and medical history. Upload lab reports, prescriptions, and other files your clinic needs.

• Membership Review your membership and complete payment when your clinic sends a link.

GETTING STARTED

1. Ask your clinic for patient portal login details if you do not have them.
2. Open the app and sign in with that email and password.
3. Use Home to reach vitals, messages, appointments, and your profile.

This app is for existing Miles Family Care patients. It does not provide emergency care. If you have a medical emergency, call 911 or your local emergency number.
