Create a clear, personalised profile for a child or adult. Record the information that matters to the individual, including:

• communication preferences and processing needs • sensory needs and signs that support may be needed • routines, preferences and reasonable adjustments • strengths, interests and important people • things that help and things to avoid • allergies, health information, mobility, eating and drinking support • emergency contacts and additional notes

Every field is optional, so the passport can reflect the person's actual needs. Update it directly on your iPhone or iPad whenever something changes.
