ISHI Health connects you with your care team through remote patient monitoring. If your healthcare provider has enrolled you in an ISHI monitoring program, this app gives you a clear view of your own health data:

TRENDS - See your blood pressure, weight, and heart rate readings from your connected devices, charted over time. REPORTS - Read the summary reports your care team prepares from your monitoring data. LEARN - Short educational videos about your health, released to you week by week.

Your readings are transmitted automatically by your monitoring devices - no manual entry, no device pairing. Supported cellular monitoring devices upload readings independently to ISHI Health. This app does not pair with or communicate directly with those devices. Your account is protected by two-factor authentication.

ISHI Health is available to patients enrolled by a participating healthcare provider. There is no self sign-up; your care team sets up your account during onboarding.
