A one-time $1.99 "Birth Certificate + Avatar" purchase is available when birthing a companion (free for paid subscribers). All purchases use Apple In-App Purchase. Subscriptions auto-renew unless cancelled at least 24 hours before the end of the current period; manage and cancel anytime in your Apple ID settings.
