Whether you have questions, need help managing your health, or want guidance between doctor visits, your care team is there to support you and help you feel more confident and in control of your health.

Your privacy is our priority – Care Elevate is fully HIPAA compliant, using secure technology to protect your personal health information. The app is provided at no extra cost to eligible members as part of your plan benefits. Getting started is easy: just sign up with the access code provided by your health plan and have your member ID (from your insurance card) handy.

Who can use this app: Care Elevate is available by invitation only. If you received an invitation code from your health insurance plan, you’re eligible to use the app.
