CareContinuum is designed to help patients stay engaged in their treatment and navigate their weight-loss care pathways without getting lost along the way.

CareContinuum provides a single place where users can track and review health information over time, including daily, weekly, monthly, and yearly measures such as weight, body mass index (BMI), physical activity and step counts, glucose, blood pressure, body fat, exercise, and sleep duration. The app can also help users find information about weight-loss medications. For a medication you select, the app may provide information about medication costs, savings cards and patient assistance programs, insurance coverage, and prior authorization requirements. In addition, the app uses Google Gemini to help identify potential barriers that may affect continued weight-loss management and to provide understandable explanations and general guidance about those barriers.

Who it’s for Adults working with a care team on overweight or obesity, including those in research or specialty programs where the app is offered.
