ConnectedNest connects individuals facing cancer to services that address needs.

ConnectedNest, a novel digital social care referral platform developed by XanthosHealth aims to connect individuals facing cancer to relevant, real-time, curated community support services that address various social needs (food, housing, financial assistance, caregiving, grief, etc). ConnectedNest has three components. For the patients, EmpowerNest is a mobile app that allows the patient to screen for their own social needs, self-refer, and connect to services. For the community organizations, CommunityNest allows organizations to list and update programs in real-time, enabling of matching patients to their services. Engage Nest is a HIPAA protected electronic health record interface for clinical teams which allows them to refer patients to social services.
