MedXcribe: Medical Scribe

MedXcribe is a private, offline medical scribe for iPhone and iPad. Record a consultation, get an accurate transcript, and have it arranged into a SOAP note — with no internet connection, and without your audio ever leaving the device.

WHAT IT DOES

Record with one tap. Start dictating from the home screen, then pause, resume, discard or save.

Watch the words appear. Live transcription runs while you record, so you can check as you speak.

SOAP notes, instantly. Your dictation is filed under Subjective, Objective, Assessment and Plan using the phrases clinicians actually use. Every sentence is kept word for word. Nothing is rewritten, summarised or invented, and a section you did not dictate is marked "Not documented" rather than filled in for you.

Timestamped transcripts. Read a note as flowing text or as timed segments, with the audio playable alongside.

Import and export. Bring in existing audio files. Copy the text, share the transcript or the SOAP note, export subtitles as SRT or VTT, or share the original recording.

Everything in one place. Every recording in a single list, each titled from what you dictated.

GENUINELY OFFLINE

The speech model ships inside the app. There is no download on first launch, no account to create, and no server involved. Put the device in airplane mode and recording, transcription and SOAP notes all still work.

Because the model travels with the app, MedXcribe is a large download. Once installed, nothing further is fetched.

MedXcribe does not use ChatGPT or any OpenAI API. Its Whisper-architecture model is fine-tuned for medical terminology and runs entirely on your device.

PRIVACY

Your audio and transcripts stay on your iPhone or iPad. There is no upload, no third-party API and no analytics service reading your notes. Optional iCloud backup is available in Settings and stays off unless you switch it on. Because nothing leaves the device by default, MedXcribe suits HIPAA-conscious workflows.

SUBSCRIPTION

The first 25 recordings are free. Unlimited recordings are available as an auto-renewing subscription at US$7.99 per month, purchased in the app. Payment is charged to your Apple Account at confirmation of purchase. It renews automatically unless cancelled at least 24 hours before the end of the current period, and you can manage or cancel it in your Apple Account settings.
