Stroke is a life changing event, yet more often than not, stroke survivors leave the hospital without knowing how to transition to life after such an event. Our mission is to bridge the gap in care between hospital and home.

Our telehealth clinical team will guide stroke survivors through their post-stroke journey in order to maximize recovery. Our HealthKit API integration lets you track your activity both manually and by bringing in data from the health app.

Please note: this app is restricted to US-based study participants at ValleyHealth in Virginia and MaineHealth in Maine. Please contact your healthcare provider at a participating site for additional information. Data collected in the study will be anonymized for use in clinic improvement research.
