DCL Heart Patient App

DCLHeart transmits vitals, symptoms and messages from Congestive Heart Failure patients to their care team, enabling timely health analysis and critical interventions. The app features wireless interfaces to several home health monitoring devices for seamless data collection and processing. It is tightly integrated to a medical console which is monitored by your health professionals. From the DCLHeart app, patients can view their trends across multiple measures over time. The app also provides interactive communication with your care team for expedient feedback and guidance. DCLHeart becomes part of a patients daily routine for health monitoring. DCL Heart connects to 3rd party devices that provide an optional method for data collection. These recommended devices which have been registered by the U.S. Food and Drug Administration for use within the United States of America. DCLHeart is made available to you by your doctor. Please consult your doctor before making any medical decisions or with any questions pertaining to the use of this application and associated devices. Jurisdiction Statement: This application, when used in conjunction with optional health devices, should be used within the USA since the health devices tested for use with this app have been reviewed by the U.S. Food and Drug Administration. Note: This application can only be enabled for patient use by a physician and associated healthcare team and can not be used indiscriminately by a casual user.

Scope of Intended Use: The DCLHeart APP is assigned to you by your physician and healthcare team to enable the remote monitoring of selected vitals and symptoms collected by you and transmitted to the healthcare team by this APP. As part of the data collection process, you can optionally use the recommended health devices which can send data to the APP using wireless bluetooth technology. You may also input your measures manually if bluetooth is not working or if you are using other health devices that are pre-approved by your physician and healthcare team.

The DCLHeart APP provides you with the following capabilities:

Daily Checklist to help ensure that the necessary data has been collected. Trend Charts which graphically display your progress over time Chat to enable you to directly communicate with your healthcare team Profile Update to allow you to make changes to your patient profile if needed

Data The DCLHeart APP does not collect personal identifiers from you, as an approved APP user. Your profile information has been preloaded by your healthcare team through their web portal. You identify yourself to the APP using your unique mobile number. You do however, have some ability to adjust or correct profile information using the DCLHeart APP. Health measures such as vitals and symptoms, are transmitted securely to your healthcare team as numeric values without personal identifiers
