Docs4D Guardian – Your Digital Companion for Connected Healthcare

Docs4D Guardian helps you record and securely share health information with your cardiologist, general practitioner, specialist, or other authorized healthcare professional.

Depending on your individual monitoring program, you can document blood pressure, heart rate, oxygen saturation, body weight, blood glucose, ECG data, and other health-related measurements. Values can be entered manually or transferred automatically from compatible, CE-marked Bluetooth medical devices.

Easy Health Monitoring

No advanced technical knowledge is required. Docs4D Guardian guides you through the recording of your measurements and helps you follow the monitoring plan defined by your healthcare provider.

You can:

• Record vital signs and health measurements
• Connect compatible Bluetooth medical devices
• View your measurement history and trends
• Complete assigned questionnaires and tasks
• Receive reminders for measurements and activities
• Securely transmit relevant information to your healthcare team

Secure Data Transmission

Your recorded health information is encrypted and securely transmitted to the healthcare organization responsible for your monitoring program.

Only authorized users involved in your care can access the relevant information. Two-factor authentication, role-based access controls, encryption, and security logging help protect your personal and health-related data.

Threshold Notifications

Your healthcare provider may define individual thresholds for selected measurements. If a recorded value is outside a configured threshold, the system may generate a notification for the responsible healthcare team.

A notification does not automatically indicate a medical emergency and does not guarantee immediate review or response by a healthcare professional.

Docs4D Guardian is not an emergency-response system and does not provide continuous real-time clinical monitoring.

Quick Activation

Docs4D Guardian is activated through a participating healthcare provider.

Your cardiologist, general practitioner, specialist, clinic, or healthcare organization enrolls you in the monitoring program and provides an activation email or invitation code.

Where required, compatible medical devices may be provided by your healthcare provider or another authorized service provider.

Who Is Docs4D Guardian For?

Docs4D Guardian may be used by patients participating in a professionally supervised monitoring program, including patients with:

• Heart failure
• Arterial hypertension
• Diabetes mellitus Type 2
• Atrial fibrillation or other cardiac rhythm disorders
• Metabolic syndrome
• Overweight or obesity
• Other conditions requiring regular monitoring of health measurements

The available functions, measurements, questionnaires, and thresholds depend on the individual monitoring program configured by your healthcare provider.
