2025-11-12	k.*****	The app is completely not functional. They have a form that is broken and cannot be submitted. Every time you login, the form pops up demanding to be filled. However, step four of the form is blank and cannot be acknowledged, yet you can’t proceed without submitting the form! Literally useless app. I’m not sure how to even access my appointment.
