My Community by Chorus

My Community provides Chorus customers with easy access to manage support visits, care plans, see what’s happening around Chorus and gives people within their chosen support network access to stay connected with their loved one's activities.

Chorus is a for-purpose organisation in Western Australia, working with communities and alongside seniors and people living with disability or mental illness. We exist so local communities can thrive. We believe in the power of relationships and connecting customers with a circle of support within their community.
